A great first appointment is easy—
Plan to arrive 15 minutes before your scheduled new patient appointment time so you’ll have some extra time to:
- Turn in or complete the new patient health history form – Link to Printable PDF
- Verify your medical insurance
- Review and sign our clinic privacy policy
If you’ve been referred by your primary care physician, please ask them to send us a copy of any relevant medical records, including lab work or x-rays, before your scheduled appointment.
Dr. Smith will then go over your new patient health history form with you and get a detailed history of your symptoms—their severity, any relation to possible allergen exposure and how you’ve responded to past treatment.
Dr. Smith will then perform a physical exam focusing on the eyes, ears, nose, throat, lungs and skin to look for signs of allergic disease–including nasal allergies, asthma and skin-related allergies such as eczema, hives or contact dermatitis.
The next step is for you and Dr. Smith to decide if any further testing is needed. This may include skin testing and or blood tests to clarify what you or your child are allergic to, lung function tests to objectively diagnose or help fine-tune control of asthma, or radiological images such as a sinus CT scan or chest x-ray. After a comprehensive evaluation of your situation, Dr. Smith and his team will develop your individualized, detailed plan of action with you.
